Comes with OG box and only 1 insert. NO MANUAL!!! The box has taken a beating throughout the years that I've owned it but no broken plastic as far as I'm able to tell. I buy don't know how I lost the manual but somehow I only have 1 insert. The disc is pretty good, it has some surface scratches and is just a tiny bit foggy but nothing major. The game works and plays to perfection. I actually just beat the game a couple months ago. Overall a fun game, I loved the art style and the combat is super well done!.
